Work-Based Learning Applications

Students and parents must complete the below form in order for students to apply for work-based learning programs associated with PCHS.

  • Part 2: Admission Minimum Requirements and Acknowledgements

    The Student Must: 1. be at least 16 years of age 2. be a Junior or Senior and on track to graduate on time 3. interview with the WBL coordinator 4. have a clearly defined career goal 5. be a student in good standing as pertaining to attendance, discipline and academics 6. have completed at least one CTAE class related to the job obtained 7. have own transportation to and from job 8. submit 2 teacher recommendations 9. be willing to submit to health/substance screenings that may be required by the employer 10. have an acceptable attendance, academic and behavior record If, after the semester begins and a student enrolled in the work-based learning program has not secured employment within the first week, the student will be required to request a schedule change and will be enrolled in regular classes with a regular class schedule. If a period of unemployment occurs within the semester, the student will be placed in an equivalent online class at the school and may not leave campus without permission of the coordinator or administrator. The student’s employer may require the student to work holidays, weekends and/or school breaks. A student enrolled in the work-based learning program may not quit a job, change jobs, or pursue a new job without prior approval from the work-based learning supervisor. In the event the student loses his/her driver’s license, other transportation will be the responsibility of the student and/or family. Students must leave campus when regular classroom instruction is complete each day. All students accepted into the work-based learning program may be required to remain on campus the first week of school during the scheduled WBL periods. I understand the minimum requirements of the Pike County Schools’ Work-Based Learning Program and failure to satisfy these requirements will prevent my acceptance into the program. Enrollment in Pike County High School’s Work-Based Learning program is limited and turning in an application for enrollment does not mean that I will be automatically accepted.
